We've just sent out a customer survey to everyone who is supported by Rise. Depending on your contact preferences, you may have already received it in a text message or email, or it could be winging its way to you right now via the post.

We'd really appreciate your time in completing this survey - it shouldn't take more than 10 minutes, and the information you provide is invaluable to us as we strive to continue improving our services. And by completing the survey, you're not only making your voice heard - you'll also go in the draw to win a $100 retail voucher!

Here are some answers to questions you may have about this survey.

Q. Why did I get the survey?

A. We want to know what we do well (so we can keep doing it) and what we can improve on. By completing the survey, your voice will be heard.

Q. Will Rise know the survey is from me? What if I say I don’t like something?

A. No one will be able to connect your responses with you. There won’t be any consequences if you provide negative feedback, as we appreciate you making us aware of an opportunity to do better. So please be honest and open.

Q. Why did I get a text message?

A. Many people prefer to be contacted via text message, and last time we did the survey, we got some feedback that the paper survey was too long and that not everyone likes writing things down by hand. But if you’d like a paper copy of the survey, please call 6274 3700 and we’ll send you one in the post.

Q. When do I need to complete it?

A. We would appreciate it if you can respond within the next two weeks. If you fill it out too late, we won’t be able to include your responses in the results.

Q: What if I haven't received my survey, or have misplaced it?

A. If your survey is being delived by mail, there may be some delays due to COVID-19. If you have concerns, speak to your Support Worker or Coordinator, or call us on 6274 3700, and we'll get you a copy as soon as we can!

Q. How will I know if I’ve won the $100 retail voucher?

A. We’ll contact you straight away and make sure it is delivered to you, for you to enjoy!
